Paid-In-Full Memberships-Members

Who purchase a Paid-In-Full Memberships have access to all their membership sessions immediately upon payment in full. These membership sessions can be used anytime if the membership is current and active. Initial Contract Agreement is for One Year.

How It Works •

Your monthly membership price will be automatically drafted from your debit or credit card each month, entitling you to a single “Any 60 Minute Session” (excluding MFR) of your choice each month, on or after your scheduled payment date. • You may upgrade to a longer session at an additional charge. • You may also share these accrued sessions with one additional member of your household. • If you do not use your session for that month, it will simply roll-over into next month. • The membership monthly fee is specified on the application above. As long as your monthly membership payment is made, you will be considered an active member in good standing.

If through no fault of ours, your payment account does not contain sufficient funds to complete the transaction, or your payment account or credit card does not otherwise permit the transaction to be executed, you will be charged a $35 insufficient funds fee. We will contact you to update your account with a working payment method. You have the right to receive a notice if we make any change to the terms and conditions of your membership that will vary the amount to be periodically billed to your account specified above. Except as expressly provided herein, we may modify our services or the terms and conditions of this Agreement at any time without notice and such modifications shall be deemed effective immediately upon making such changes.

MEMBERSHIP TERMS & CONDITIONS:

This membership and services purchased under this membership are non-transferable to any other person or entity. For purposes of identification and billing, you agree to provide us with current, accurate, complete, and updated information including your name, address, telephone number and applicable payment data. You agree to notify us promptly of any changes. We reserve the right to revoke membership at any time for misconduct on our premises. You will be responsible for payment in full upon revocation of membership. We reserve the right to change pricing at any time upon reasonable notice. Should prices increase, you have the right to cancel membership without penalty during the initial period following cancellation guidelines as described below.

CANCELLING YOUR APPOINTMENT:

You may cancel your appointment without charge if you cancel within 24 hours of your appointment. Cancellations without 24-hour notice may result in a $25.00 charge for your session, as that time has been set aside specifically for you. No shows may result in a full charge for that session.

CANCELLATION OF MEMBERSHIP DURING INITIAL TERM:

You may cancel this agreement during the initial term of your membership upon the following conditions: Your death or permanent physical disability, or if you move out more than an hour away. Written notice must be given at least 30 days (about 4 and a half weeks) in advance to avoid additional charges to your account.

Contracted membership services will be forfeited at time of cancellation effective date.

CANCELLATION OF MEMBERSHIP DURING AUTO RENEW TERM:

After the initial 12-month term, you may cancel at any time. All cancellations require 30 days (about 4 and a half weeks) advance written notice or email and are effective within 10 business days after the 30-day notice period. Payments due prior to the effective date will be charged as scheduled.

Pre-paid membership services have no cash value, are non-transferable, and services expire 12 months from date of purchase according to agreed terms. We reserve the right to terminate or deny re-enrollment for an indeterminate amount of time if you have an unsatisfactory payment history.

NOTES ABOUT YOUR MEMBERSHIP:

• At Partner To Heal, a 1-hour session consists of a FULL 60-minute session time.

• Additional time for consultation and dressing is free of charge.

• You have the entire length of your membership to use your membership services if your account is not frozen and is in good standing. Any unused services within the 12-month agreement period will expire should you elect to cancel your membership or decide not to renew your membership.

• Partner To Heal Memberships are Non-refundable and Non-transferable.

• All Memberships are auto Renewable. At the end of the initial term of membership, your dues will automatically renew on a month-to-month basis Unless you have elected otherwise on the Membership agreement.

• Memberships may be shared with one other family member who resides in the same household.

• Partner To Heal reserves the right to change office rules, regulations, and pricing at any time upon providing reasonable notice.

• You agree to follow office rules and regulations. We have the right to refuse or discontinue service for any reason.

PAID-IN-FULL MEMBERSHIPS:

Members who purchase a Paid-in-full Membership have access to all their membership sessions immediately upon payment in full. These membership sessions can be used anytime if the membership is current and active.

12 MONTH MEMBERSHIP CONTRACT – MONTHLY INSTALLMENTS:

Massage membership monthly payments are automatically debited from the member’s registered credit/debit card. The payment is debited monthly on the same day of each month as the first payment (this is the sign-up date). The monthly charges will continue automatically each month for 12 months, or until the termination of the membership.

EXAMPLE:

Sign-up/First Payment on 10/11/23

o First Automatic Payment charged on 11/11/23

o Final Automatic Payment charged on 9/11/24

o Membership benefits continue until 10/10/24 (the expiration date).

FREEZING A PARTNER TO HEAL MEMBERSHIP:

There may be times when you would like to freeze your membership due to extenuating circumstances, such as extended illness, extended vacation, military leave or temporary relocation.

• When a membership is frozen, a member may not use his/her membership benefits or accrued membership services for the duration of the freeze period.

• A minimum of one membership payment must have been made before a membership is eligible to be frozen.

• You may freeze your membership for a minimum of 30 days and a maximum of half of your original membership term. For example, a member with a twelve-month membership term may freeze their membership for up to six months.

• Memberships may be unfrozen at any time at least 30 days after the start of the freeze or will automatically recommence on the pre-determined freeze end date. At the end of the membership freeze term, your dues will continue with your next scheduled electronic funds transfer (EFT) date. Remember that you must be a member for a minimum of 31 days before you can freeze your membership.

• A membership must be unfrozen a minimum of 30 days before it can be frozen again.

• Your membership expiration date will be extended by the amount of time that your membership was frozen.

• Terms, conditions, rules and regulations of the original Membership Agreement continue to apply through the extended membership term. To initiate a freeze, contact clinic management.

EXAMPLE:

o Sign-up/First Payment on 10/11/23.

o First Automatic Payment charged on 11/11/23.

o 60 Day freeze is requested on 5/13/16 and processed on 5/23/24.

o Automatic payments scheduled between 5/23/24 and 7/21/24 will not be charged, this would include regular installments scheduled to take place on 6/11/24 & 7/11/24.

o Membership benefits would also be suspended from 5/23/24 until 7/21/24 and would resume on 7/22/24.

o The next automatic payment would then be charged on 8/11/24.

o Final Automatic Payment charged on 11/11/24.

o Membership benefits continue until 12/10/24 (the expiration/auto-renew date).

TERMINATION OF MEMBERSHIP:

To cancel this membership prior to the end of the contract, a $30 termination fee is required. All requests for membership cancellation must be submitted to Info@PartnerToHeal.com at least 10 days in advance. Once we have received and completed your request and $30 cancellation fee, the next scheduled monthly Page 6 charge and all those that were to follow, will be cancelled. Membership will be valid until, but not on, the date of the first cancelled payment; after that date, all member benefits, including unused accrued services, will also be cancelled.

EXAMPLE:

o Sign-up/First Payment on 10/11/23.

o First Automatic Payment charged on 11/11/23.

o Membership cancellation is requested on 5/13/24 and processed on 5/23/24

o A $40 cancellation fee will be charged on 5/23/24, the date of cancellation.

o The automatic payment scheduled for 6/11/24 and all subsequent automatic payments will NOT be charged.

o Membership benefits continue until 6/10/24 (the expiration date)

Partner To Heal Policies

Therapeutic massage is intended to enhance relaxation, reduce and/or eliminate pain, increase range of motion, improve circulation, and offer a positive experience of touch. The massage therapist does not diagnose illness, disease, or any other physical or mental disorders. As such, the massage therapist does not prescribe medical treatment or pharmaceuticals, nor do they perform spinal manipulations. Massage is not a substitute for medical treatment or diagnosis, and it is recommended that I see a physician for any physical ailments that I may have. None of the information provided is intended to act as a substitute for medical advice, nor does it involve the diagnosis, prognosis, or prescription of remedies for the treatment or prevention of any disease or ailment.

o It is the responsibility of the client to keep PTH Therapist informed of any current medical conditions or any changes in health and provide written permission (if under care) from a physician to the PTH Therapist prior to each session.

o The client will come to the massage therapy appointment clean.

o If any pain or discomfort is experienced during the session, the client will immediately communicate with the Therapist so the treatment can be adjusted.

o Both the therapists and client are vulnerable to infections. Therefore, clients and the therapist are asked to reschedule appointments when they are feeling unwell.

o For the comfort of the therapist and other clients who may have allergies, please refrain from wearing perfume or fragranced products to your appointment.

o Payment is due in full in advance or at the time of service via cash, check, Venmo, Zelle or credit card.

o 24-hour advance notice is required when canceling an appointment, except in cases of illness, emergency, or inclement weather. Cancellations without 24-hour notice may result in a charge for your session, as that time has been set aside specifically for you.

o Please arrive on time for your appointment. Time for your appointment has been arranged for you. If you arrive late, your session may be shortened to accommodate others whose appointments follow yours. Full payment for your scheduled session will be expected.

o Sexual advances of any kind will not be tolerated, and the session will be terminated immediately.

Cancellations without 24-hour notice may result in a $25.00 charge for your session, as that time has been set aside specifically for you. Discounted offers, promotions, packages, specials, or existing discounts of any other kind cannot be combined with any other discounts. Should other current promotional discounts or offers turn out to be a better deal, we will happily do our best to ensure your satisfaction.
